JLL selected to sell 261 Grand Concourse

 JLL Capital Markets was selected by  
 undisclosed clients to sell 261 Grand Concourse. 
   The  10,242-square-foot  development  
 site is located in Mott Haven, and the  
 sellers are asking for $12.5 million. 
 Karl  Brumback,  managing  director;  
 and Brian Streilein and Eric Tiefen, associates, 
   make  up  the  JLL  group  handling  
 this project. 
 The property at 261 Grand Concourse  
 offers  between  61,657  square  feet  and  
 73,742 square  feet of development rights  
 with  potential  for  residential,  hospitality  
 and  commercial  uses.  The  current  tenants  
 are  all  on  short-term  leases,  which  
 are set to expire in June 2021. The site is  
 located in a Qualifi ed Opportunity Zone at  
 the busy corner of Walton Avenue, Grand  
 Concourse  and  East  138th  Street  in  the  
 borough’s  Mott  Haven  neighborhood,  
 which is the Bronx’ most active development  
 zone. Approximately 6,500 new residential  
 units  are  coming  to  the  area  and  
 several large scale development projects  
 are underway nearby.

 National  tech  training  nonprofi t  Per  
 Scholas  announced  the  appointment  of  
 Abe Mendez, current deputy managing director  
 for Newark & New York, as managing  
 director  of  New York.  The  leadership  
 change comes as longtime managing  director  
 Kelly Richardson concludes her  
 time at Per Scholas, following a successful  
 six-year tenure that included the launches  
 of  the Per Scholas Brooklyn and Newark  
 training centers, as well as  the  launch of  
 new cybersecurity, cloud, and web development  
 training tracks.  
  In Kelly’s six years at Per Scholas, she  
 oversaw a team enrolling nearly 3,000 individuals  
 across Per Scholas’s New York  
 area  training program and built new support  
 services  for students while  fostering  
 strong  community  and  corporate  partnerships, 
   such  as  serving  as  co-chair  of  
 NYC’s  Employment  and  Training  Coalition. 
   
  As managing director overseeing Per  
 Scholas’s campuses  in Brooklyn and  the  
 Bronx, Abe will focus on driving additional  
 site  expansion  efforts,  meeting  ambitious  
 programmatic and fundraising goals  
 within the New York region, while building  
 existing  and  new  corporate  partnerships  
 to support Per Scholas graduates.  
 Prior  to  joining  Per  Scholas,  Abe  
 served as an assistant vice president at  
 Barclays in New York and London where  
 he  helped  manage  the  bank’s  citizenship  
 partnerships and efforts to engage  
 colleagues  in  skills-based  volunteering  
 programs  and  campaigns.  During  
 his  time at Barclays,  Abe was a driving  
 force in  the company’s partnership with  
 Per Scholas to support the launch of Per  
 Scholas  Brooklyn  and was  a  partner  in  
 its development  of  a new  cybersecurity  
 training track. He also worked at JPMorgan  
 Chase & Co.  in  a  variety  of  operations  
 and program management roles.

 the  2019-2021  seasons.    This  sponsorship  
 allows Healthfi rst  to  reach our  audience  
 in  channels  it  has  not  used  in  the  
 past such as  in-stadium, on  radio and  in  
 promotions. 
 Now in its second year as the “Offi cial  
 Wellness Partner of Citi Bike,” Healthfi rst  
 will  update  the  branding  on  docking  stations  
 to  incorporate  the  new  campaign  -  
 particularly  those  near  NYCHA  housing  
 where Reduced Fare Bike Share membership

 NYC  Health  +  Hospitals’  Offi ce  of  
 Behavioral  Health,  in  partnership  with  
 OneCity  Health,  announced  the  launch  
 of Pathway Home, an innovative care transition  
 program  created  by  Coordinated  
 Behavioral Care  (CBC),  to  help  individuals  
 with  mental  health  needs  transition  
 from  short-term health  services  to ongoing  
 community-based care. The Pathway  
 Home  program  will  provide  multidisciplinary  
 care transition services to patients  
 18 and older transitioning from psychiatric  
 inpatient units who have had four or more  
 mental health inpatient visits in the past 12  
 months. Pathway Home teams will actively  
 participate  in aftercare planning and perform  
 a transition needs assessment, accompany  
 patients home and arranging for  
 any  immediate  needs,  such  as  food  and  
 fi lling  prescriptions,  accompanying  them  
 to  primary  care  visits,  and  meeting  with  
 them regularly for six to nine months postdischarge. 
   In  addition  to  reducing  avoidable  
 readmissions, Pathway Home aims  
 to strengthen community-based care and  
 improve  connections  to  outpatient  services  
 post-discharge, shortening length of  
 stay and  increasing participation  in  treatment. 
 OneCity  Health  is  investing  over  $4  
 million in the Pathway Home program and  
 providing  technical  assistance  as  part  of  
 its effort to implement new models of care  
 that will reduce avoidable hospital utilization  
 under New York State’s Delivery System  
 Reform  Incentive  Payment  (DSRIP)  
 program.  The  investment  will  help  fund  
 the new services for NYC Health + Hospitals’ 
  mental health patients through a contract  
 with  Coordinated  Behavioral  Care  
 and additional partnerships  with  community 
 based  organizations  (CBOs).  Pathway  
 Home will launch at four NYC Health  
 + Hospitals acute care facilities. Each site  
 will  work  with  one  Pathway  Home  team  
 and will  enroll  up  to  150  patients  over  a  
 15-month period.  The four sites are: NYC  
 Health  +  Hospitals/Coney  Island;  NYC  
 Health + Hospitals/Harlem;  NYC Health +  
 Hospitals/Lincoln; and NYC Health + Hospitals/ 
 Metropolitan.
