To make the process of receiving these services as smooth as possible, the Care Managers can provide authorization to begin receiving services or to continue receiving services. AgeWell understands that it can be complicated at times to receive proper assistance, and that is why the organization makes it easy to reach a Care Manager at any time of the day. Your Care Manager is available to answer questions Monday through Friday from 8:30 a.m. until 5 p.m., but if you need assistance outside of those hours your call will be forwarded to another member of the team, so there will always be someone available to help. “I’m most proud of the team and making the member experience,” said Buonocore-Rut. “I think you really have to believe in anything that you do, and they enjoy it.” The services that AgeWell provides cover a wide range of care coordination needs—from the very basic day-to-day personal care, to transportation, to longerterm nursing home care, if necessary. Care Managers can help facilitate in-home nursing, medical social services, physical, occupational, and speech therapies, obtaining medical or surgical supplies, Personal Emergency Response Systems (also known as PERS), non-emergency medical transportation, podiatry, optometry, and dental services. Each AgeWell member is assigned a Care Manager, who caters to the different medical needs of each individual. including bathing, eating, dressing, and social support. From the very first steps, AgeWell works to make the enrollment process easy, thorough, and stress-free. Then, an AgeWell nurse will schedule a home visit to do a full assessment of the person’s needs and review the services that they currently receive. At this time, the person seeking care can ask any questions that they may have about AgeWell’s coverage and policies. The nurse will explain the benefits that AgeWell offers, provide a copy of the Member Handbook and Directory of Providers, and outline a personalized care plan based on an individual’s health and needs. If necessary, the AgeWell nurses can also assist with completing AgeWell enrollment agreements or Medicaid applications.
