With the help of the professional staff at AgeWell New York, a managed longterm care plan, people with disabilities and people with chronic illnesses can receive the help that they need to stay in their homes and their communities for as long as possible. AgeWell aims to provide each adult in its network with choices and flexibility to make daily life safe and comfortable. “We try to be the glue that puts the health care pieces together,” said Tara Buonocore-Rut, Executive Director. AgeWell New York works with patients in Queens, Brooklyn, Manhattan, the Bronx, Westchester, and Long Island’s Nassau and Suffolk counties. The organization serves individuals who are over the age of 21, are able to stay safely at home at the time they enroll, are covered by or eligible to apply for Medicaid, are eligible for the nursing home level of care at the time they enroll and are expected to need long-term care services for at least four months. “Long-term care is the future,” said Buonocore- Rut. “We want to target membership satisfaction, and make sure members who enroll with us, stay with us.” From the moment someone signs up, the “unique” AgeWell staff works directly with the patient, their family, and their healthcare providers to ensure clear communication regarding health care and personal care needs. AgeWell’s Care Managers are highly trained in the healthcare field and know the services available in New York City and its surrounding areas. Because the Care Managers help connect patients with services and healthcare providers located in their communities, AgeWell New York eliminates the challenge of having to travel far distances to utilize necessary services.
