Guest speaker Stacy Mallin from Healthcare Association of New York State Solutions will discuss Patient-Centered Medical Homes, a teambased, coordinated care model used to organize primary care. Medical homes are intended to improve quality, lower costs, and improve patients’ and providers’ experience of care. Nine new NewYork-Presbyterian/Queens programs are being developed to address mental health, asthma, cardiovascular and HIV treatment needs, as well as integration of palliative care, home care and other areas. The DSRIP program goal is to establish an integrated health care delivery system that can effectively and efficiently meet the needs of our community.
