Potentially eligible children must be referred to the County program in their area to receive EIP services. To refer a child to the EIP in NYC call 311; in Nassau 516-227-8661; in Suffolk 631-853-3100; in Westchester 914-813- 5094), or call the NYS Growing Up Healthy Hotline at 1-800-522-5006. The EIP is funded by the NYS DOH and County DOH’s. All EIP services are At No Cost to parents. Health Insurance is not required, but may be assessed by the county for approved EI services. A child’s eligibility for the program is based on eligibility guidelines established by NYS DOH . Children can only be evaluated only by a state approved sites. The County is responsible for approving services. Preschool: Parents concerned about the development of their preschool child ages 3 -5 may refer their child to their School District’s Committee for Preschool Special Education {CPSE} to request an evaluation. Only the CPSE determines eligibility and authorization of services. The Committee on Preschool Special Education is funded/monitored by NYS DOH & NYS ED. Dept., as well as County governments.
