Guest speakers will be Elvira Fardella-Roveto, RN, FNP-BC, administrator and director of professional services for St. Mary’s Home Care, and Donna Mapp-Reid, RNC, CCM, supervisor for St. Mary’s Telehealth Program. They will discuss the initiatives related to pediatric asthma education. This Town Hall will provide a progress report on the NYP/ Queens PPS programs overall. The nine NYP/Queens PPS programs address mental health, asthma, cardiovascular, and HIV treatment needs, as well as integration of palliative care and home care. The DSRIP program goal is to establish an integrated, high- performing health care delivery system that can effectively and efficiently meet the health care needs of our community while transforming care, increasing access, improving outcomes and reducing costs.
