Assist with Nursing Home Placement:
a. Refer to appropriate Nursing Home Facilities.
b. Coordinate Nursing Home Placement.
c. Arrange for Patient Review Instrument (PRI)
d. Working relationship with Nursing Homes.
Process Medicaid Applications:
a. Assess Medicaid eligibility requirements.
b. Review financial information.
c. Gather appropriate documentation.
d. Prepare, coordinate and submit the Medicaid application.
e. Follow-up with caseworkers.
f. Working relationship with Department of Social Services throughout Suffolk, Nassau and New York City.
Process Pooled Income Trust:
a. Explain procedure of Pooled Income Trust.
b. Complete forms and documentation.
c. Prepare, complete and submit paperwork.
d. Coordinate paperwork with Department of Social Services throughout Suffolk, Nassau and New York City.
Referral Resources:
a. Provide access to Financial Planners, Elder Law Attorneys and Geriatric Care Managers.
