 to  schedule  an  appointment  
 at any of  the  city’s offered  locations.

 • Please note: If you book  
 an appointment through the  
 New York State website and  
 are able to secure an earlier  
 appointment at an alternative  
 site like a pharmacy, you can  
 simply cancel your later appointment  
 and receive your  
 vaccine  at  the  site  of  your  
 choosing. 
 At this time, there are no  
 plans to provide a Vaccination  
 Center at the airport..

 Port Authority Bulletin Information 
 As of January 11, 2021,  
 New York State has moved  
 into Phase 1B of the COVID-19  
 Vaccination  Program.  This  
 means that public transit  
 workers who regularly report  
 to work, including Port Authority, 
  Airport and Airline  
 employees, are now eligible to  
 receive the vaccine. Eligible  
 employees in Phase 1B can begin  
 scheduling appointments,  
 though space may be limited  
 during the initial launch of  
 this new phase. 
 There are two steps to  
 take before you schedule an  
 appointment: 
 1. Determine eligibility  
 here:  https://am-i-eligible.covid19vaccine. 
 health.ny.gov/.  
 Remember to self-identify as  
 a Public Transit Worker and  
 then  select  the  appropriate  
 sub-category in the dropdown  
 box. 
 2. Complete the New York  
 State COVID-19 Vaccine Form  
 here: https://forms.ny.gov/ 
 s3/vaccine . Successful completion  
 of the form will be  
 followed by an email  confi rmation  
 and Submission ID  
 Number. Please bring this  
 with you to your appointment. 
 Port  Authority,  Airline  
 and all other Airport employees  
 who have been reporting  
 to  their  work  locations  (not  
 working remotely) should  
 sign up as soon as possible to  
 receive the vaccine. Please

 to be vaccinated. 
 Reminder: 
 When asked if you are a  
 Public Transit Worker, check  
 yes. You will then be asked to  
 specify what type; check off  
 Airport,  Airline  or  Port  Authority. 
 In addition to the Submission  
 ID Number, individuals  
 being  vaccinated  must  bring  
 proof of eligibility (public  
 transit employment) to the  
 vaccination site. This may include  
 an employee ID card, a  
 letter from an employer or affi  
 liated organization, or a pay  
 stub, depending on the specifi  
 c priority status. 
 Additional Information: 
 • New York is adding thousands  
 of distribution sites  
 including 500 pharmacies,  
 which will come online this  
 week. Additionally, 20 mass  
 distribution sites will become  
 available over the next several  
 weeks including the Javits  
 Center,  which  will  open  on  
 Wednesday. To fi nd  out more
