1) BY ABSENTEE: You must request your absentee ballot by mail, phone, or online. On the form,
mark "Temporary Illness or Physical Disability" (Question 1), fill in your DOB (Q4), and sign/date the
application (Q8). OR call 718-730-6730 to request a ballot; OR go online: nycabsentee.com.
Applications must be postmarked, requested by phone, or completed online by JUNE 16. The Board
of Elections will then mail you the absentee ballot.
Use both envelopes when returning your completed absentee ballot. Place your completed
ballot in the "Official Absentee Ballot" envelope. Seal and sign in the space provided under the seal.
Place that sealed envelope inside the larger, postage paid envelope. Your envelope must be
postmarked by Monday, JUNE 22.

3) PRIMARY DAY: Tues., June 23, at your regular polling site. Mask required.
