AgeWell New York names
new executive director
and other key promotions
Chairman of the Board of Managers
for AgeWell New York, Michael N.
Rosenblut, was offi cially name Patricia
Connelly, RN, AgeWell New York’s
new executive director.
Patricia, a Registered Nurse for
over 25 years and veteran in the
healthcare industry, has been with the
health plan since its inception in 2012
and has been responsible for leading
the successful growth of the Medicaid
Managed Long Term Care Program
(MLTC).
The MLTC plan helps those with
chronic illness and disability needing
health and long term care services
and has a special focus on care management
and supporting members’ access
to benefi ts across the continuum
of care.
“Patricia has been a key part of
growing AgeWell New York’s membership
and has always portrayed exemplary
leadership qualities. We are
thrilled to have her expertise guide us
in the company’s future growth,” said
Rosenblut.
Other notable key employee promotions
include Joyce Little, RN, named
associate executive director and Justin
Walter as assistant executive director.
Joyce and Justin have also been
at AgeWell since the plan’s inception
and have been infl uential in leading
AgeWell New York’s evolution in the
healthcare industry.
Joyce plays an integral role in leading
the Care Management team to provide
a coordinated plan of care across
multiple care settings to ensure member
have full access to seamless, high
quality health care, and to make the
health delivery system as effi cient as
possible. Justin is responsible for leading
daily operations and technology
services to bring aggressive practice
strategies to optimize health care coverage.
Under Patricia, Joyce and Justin’s
leadership and strategic insights, the
plan has achieved overall four stars
and fi ve stars rating for Care Management
refl ected in the 2018 MLTC Consumer’s
Guide.
AgeWell New York is a managed
