Routinely families call when an immediate need for
care is imminent. Under these circumstances, stress

Discharge from the hospital involves the case
management team to arrange for the continuum of
care. Oftentimes rehabilitation is necessary.
Medicare will usually cover short-term care;
however, it is important to check your plan to be
certain. If discharge plans are steering your loved
one directly home, Medicare can cover the cost of a
certified aide (up to 20 hours per week), if a skilled
service in the home (physical and/or occupational
therapy, wound care, etc.,) is authorized. If more
aide assistance is required, arrangements can be
made beyond the Medicare-approved hours where
payment would be the responsibility of the patient,
either through a long-term care policy or
out-of-pocket.
