residence, cause of action is abandonment.
You are summoned to appear
in this action by serving a Notice
of Appearance on the Plaintiff's

NY.10007, tel.(917)215-0850 within
30 days after service is completed,
and if you fail to appear, judgment
will be taken against you by default.
To the above named Defendant, the
Summons is served upon you by
Order of Hon.Jodi Orlow, a justice
of this court, granted on January
14, 2022. The nature of the ancillary
relief demanded is: The Plaintiff
may resume use of her maiden
name, Diallo, or any other former
surname. The provisions of DRL
Sec.236 Part B Section 2 (Automatic
Orders Notice), DRL Sec.255
(Health Insurance Notice), Notice
of Guideline Maintenance and Notice
of Electronic Filing shall apply.
The Defendant shall pay the sum of
$25.00 per month to the Plaintiff for
child support. The court shall grant
such other and further relief as it
may deem just and proper. The relief
sought is a Judgment of Divorce.
