AgeWell New York Moves to Expand Services in Bronx Community

On Monday, May 3, Preston High
School welcomed back student cohorts
Team Dignity and Team Honor
for daily, combined in-person classes.
The administrative team has worked
diligently to methodically assess
mitigation methods to maintain a
healthy school environment.
The decision to combine Team
Dignity and Team Honor was based
on feedback from several of families
and faculty. Currently, each attending
cohort class size averages between
5 to 7 students per day. Combining
the cohorts will result in an
average class size of 15 students. The
students will continue to wear masks
and adhere to the CDC recommended
social distancing guidelines. Sanitizing
protocols will remain a priority
as the school continues to welcome
back its students.

As an Act of Kindness, On May 12,
Pamela Johnson and Young Kings &
Queens Chess Club presented parents
who were picking up their children
from school in front of 15with
fl owers, balloons, and gift bags. The
mothers, who were surprised and
very thankful, said it was refreshing,
given this arena, to see smiles
and faces fi lled with gratitude.
May 11, 2021 –AgeWell New
York is excited to announce a
move to a new, larger location at
1120 Morris Park Avenue in the
Bronx. The New York State Managed
Care Plan delivers value in
health care coverage and care
management services through
its Medicare Advantage Prescription
Drug Plans and Managed
Long-Term Care plans. The new
offi ce will provide AgeWell staff
with much-needed space to host
provider, member, and community
engagement events in the future.
“Expanding our presence will
enable us to reach a larger scale
of potential markets ranging
from community partnerships
to healthcare professionals, who
will work hand-in-hand to help
current and future members live
well, feel well, and age well in New
York,” said Michael N, Rosenblut,
Chairman of the Board of Managers
for AgeWell New York.
In addition to locations on
Long Island and Manhattan,
the new Bronx offi ce will help
strengthen the AgeWell New
York footprint in the community
and provide additional healthcare
coverage options to Bronx
residents. In 2020, the company
launched the AgeWell New York
Advantage Plus Plan (HMO DSNP),
a Medicaid Advantage Plus
(MAP) program designed for people
who have Medicare and Medicaid
that need health and longterm
care services and supports
in the home.
“Our goal is to help the Medicare
and Medicaid populations
navigate and better understand
their health care coverage options
to make well informed decisions.
The Bronx offi ce provides
an opportunity for us to reach
more diverse populations while
giving seniors in the community
easier access to our services
including disease management
programs and educational resources,”
said Patricia Connelly,
RN, AgeWell New York’s Executive
Director.
AgeWell New York’s team of
nurses, social workers, and wellness
coaches work closely with
doctors and specialists to provide
a coordinated plan of care to ensure
members have full access
to seamless, high quality health
care. The Bronx location will
also help support the growth of
AgeWell New York’s in-network
providers and hospitals in the
New York metropolitan area.
